Berkeley County 4-H All Stars
WV All Star Nominations – Due April 1, 2017
- Constitution & Bylaws – rules for who is eligible, initiation, executive committee, etc.
- Leader Form – A volunteer 4-H leader must be at least age 18 as of January 1 of the current year, must currently be serving as a 4-H leader and must be serving in at in least their fifth year as a 4-H leader.
- Member Form – A 4-H member must be currently enrolled in at least their fifth year as a 4-H member; must have been awarded the 4-H pin; must be at least seventeen (17) years of age on January 1 of the current year recommended and have not reached their 22nd birthday by January 1 of the year recommended.
- Executive Committee – the list of committee members and their roles.
~ Recommendation forms not arriving at WVUES – Berkeley County 400 W. Stephen Street, Martinsburg, WV by April 1, 2017 are not considered.
~ No more colored paper. All forms can be printed on white.
~ Please place the page with the Membership Due Card at the back of the nomination form.
~ Use only 2017 forms.
~ If you are resubmitting a previously approved nomination form, you must leave the old cover sheets with previous chief signatures attached. If you don’t, she will have to hunt you down. This is very important!!!!
Forms must be TYPED
Do not change the font size or spacing. This is very important on the ½ page summary that is read at the consecration. BRIEF SUMMARY. If you write too much, they will only highlight and read parts of it. Plus if the font is too small, they have to enlarge the page for the reader to be able to see the words.
